A Keck Medicine of USC study has found that two years of high-dose fish oil supplements did not improve memory, cognitive function or brain-cell loss in older adults at elevated risk for Alzheimer’s, even though the key omega-3 reached the brain.
The result matters now because Americans spend more than $1 billion a year on fish oil supplements, often on the belief that omega-3 fatty acids support brain health. In this Hussein Yassine USC study, 365 adults ages 55 to 80 who rarely ate fish were randomly assigned to daily fish oil capsules or a placebo in a placebo-controlled, double-blinded trial published in eBioMedicine.
The supplement delivered 2,000 mg of docosahexaenoic acid, or DHA, and after six months DHA levels in cerebrospinal fluid rose by an average of 17%. But the gain did not translate into a measurable benefit where it counted. After two years, the DHA group did no better than the placebo group on memory and cognitive tests, and brain scans showed no protection against shrinkage of the hippocampus.
That disconnect is the heart of the finding. The brain took in more DHA, yet the expected payoff never arrived. About 47% of the participants carried APOE4, a genetic risk factor tied to Alzheimer’s, which gave the study a population that was already more vulnerable than the typical supplement user. It also means the result is not about whether omega-3s can be absorbed at all. They can. It is about whether that absorption is enough to change brain outcomes in older adults already at risk.
Hussein Naji Yassine said the team is working to understand how the brain processes omega-3s and whether poor health, dietary pattern, genetic risk and age may change the brain’s ability to absorb and use them. He said the group is also developing medications that may help the brain better use these nutrients to preserve cognitive function. He said there is no silver bullet for preventing Alzheimer’s and that the findings do not support fish oil supplements as a preventive measure.
The broader picture is narrower than the supplement aisle suggests. Omega-3s still matter in brain biology, and researchers say they may work better as part of a Mediterranean-style diet than as a standalone pill. But this trial suggests that simply taking DHA is not enough to protect brain health in older adults at elevated risk for Alzheimer’s. The next question is whether the brain can be taught to use omega-3s differently.
